Insurance Reimbursement Procedures
Coverage may be available for PT

You are responsible for payment at the time of your physical therapy sessions. Please stop at the front desk after each visit. You can pay by the session or buy a package of five or ten sessions. For pricing information, see the front desk.

Call your insurance company prior to your initial visit to determine your physical therapy benefits. Let them know that you need reimbursement information regarding "out of network" physical therapy coverage. They will explain your reimbursement options.

Some insurance companies require a physical therapy prescription/referral and/or preauthorization to process your reimbursement. If they require preauthorization, they will need a doctor's referral. Ask your insurance company if they require either one of these.

When you have completed your physical therapy course of treatment, you are ready to submit paperwork to your insurance company for reimbursement. Please call or email Kirsten prior to your final appointment so she can prepare the forms. (Phone: 503.646.8482, or kirsten@adapttraining.com)
